Dr. Fazio received his board certification in Otolaryngology in 1982, a year after partnering with Drs. J. Aaron Roy and William Dimattia to establish the Baton Rouge Clinic of ENT. Dr. Fazio is a graduate of LSU Medical Center in New Orleans. His extensive five years of post-graduate training brought him to Charity Hospital in New Orleans, LSU's Kresgie Hearing Research Lab, and the VA Medical Center of New Orleans.

Dr. Fazio is on active staff of Our Lady of the Lake Regional Medical Center, Baton Rouge General Health Center, Surgi-Center of Baton Rouge and the Baton Rouge Ambulatory Surgical Center. He is a member of the American Board of Otolaryngology, the American College of Surgeons, AMA, LSMA and the EBRP Medical Society. Since 1988 he has been a delegate to the Louisiana State Medical Society from East Baton Rouge Parish Medical Society.
